Have you ever wanted to know exactly what Social Security benefits you receive, how many trial work months you have used and/or what the status of your Medicare/MassHealth is? Well, Social Security has answers for you in the form of a Benefits Planning Query (BPQY), also known as Form SSA-2459. The BPQY will provide you with a snapshot of your benefits and work history, as it is stored in Social Security’s electronic records.
If you are interested in obtaining your BPQY, you can do so by calling your local Social Security office or Social Security’s toll-free number at 800-772-1213. It will be mailed to your address as shown on Social Security’s records.

Why is it important to obtain all this information? First, it will allow you to see what Social Security has in its records about you. If you see any mistakes you can call Social Security and correct your record. Second, you will need a BPQY when you meet with your benefits specialist. This will ensure that you get a comprehensive benefits analysis in a timely fashion. Finally, your benefit specialist will be better able to answer your questions when he/she has accurate information.
Remember to bring your BPQY to your appointment with your benefits specialist!
