Special Note:Providing Documentation of Citizenship for Medicaid

A new federal law is in place to obtain Medicaid (in Massachusetts we call our Medicaid program MassHealth, so from now on in this article we will refer to Medicaid as MassHealth). If you already have MassHealth or are applying for MassHealth you will be asked to document your U. S. citizenship and identity. However, if you are enrolled in Medicare or receive Supplement Security Income (SSI), or are a “Qualified Alien,” this new law will not affect you.

The best way to show your citizenship and identity are:

  • A U.S. Passport, or
  • A Certificate of Naturalization (DHS Forms N-550 orN-570), or
  • A Certificate of U.S. Citizenship (DHS Forms N-560 or N-561)

If you don’t have the above papers you can:

Document citizenship with

  • Your birth certificate, or
  • A report or certification of birth abroad of a U.S. Citizen, or
  • U.S. Citizen I.D. card, or
  • Adoption records, or
  • Military record

And then document your identity with

  • Your picture on your current drivers license or State identity card, or
  • School identification card, or
  • Federal, State, or local identification card, or
  • U.S. Military identification card if it shows where you were born.

Even if you don’t have any of the documentation listed above, there are other ways to show MassHealth your identity and your U.S. citizenship. MassHealth is instructed to assist people with disabilities in finding those documents.
